Tragic Incident Unfolds at Summerlands Hospital
The recent inquest into the tragic death of Jessica Powell, a 20-year-old from Yeovil, Somerset, has shed light on a series of failures at Summerlands Hospital that led to her untimely passing. Jessica, who was awaiting specialist treatment for emotionally unstable personality disorder (EUPD), met her demise in August 2020 after attempting to abscond from the psychiatric ward.
The Unfortunate Sequence of Events
According to the jury’s findings, Jessica became trapped in a therapy room window while trying to escape from the hospital. Staff negligence played a significant role in her death, as they failed to adequately supervise and secure the room where Jessica was held. The faulty alarm system, which directed staff to the wrong location, and the lack of access from both exterior and interior doors further hampered rescue efforts.
A Heartbreaking Loss for the Powell Family
Jessica’s father, John Powell, expressed profound sadness over his daughter’s passing, attributing it to an “error of judgement.” He highlighted Jessica’s vulnerability and her struggle to fit into the real world, emphasizing her kind and loving nature. Jessica’s sister Lucy remembered her as a devoted auntie, dearly missed every day.
Lessons Learned and Actions Taken
In response to Jessica’s tragic death, Somerset NHS Foundation Trust has initiated an independent report to review her care and treatment, as well as the ward’s layout, to address the family’s concerns. While expressing deep regret over the incident, the trust’s service group director for mental health, Jane Yeandle, emphasized the importance of implementing recommendations from the report and the inquest’s conclusions to prevent similar occurrences in the future.
